Eligibility & Requirements

To ensure Alpha Care is the right fit, we look for the following criteria:

👤 Age & Demographics

Adults aged 18+ with intellectual disabilities, mental health challenges, or needing transitional support.
📋 Stability

Must be medically stable and not require 24/7 skilled nursing care (we are a non-medical facility).
🤝 Behavioral

Must not pose a danger to themselves or others. We conduct individual assessments for safety.

Application Checklist

✅ Valid ID / Social Security Card
✅ Proof of Income / Benefits (SSI, SSDI, etc.)
✅ Recent Medical History / Medication List
✅ Guardianship Paperwork (if applicable)
✅ COVID-19 Vaccination Record (recommended)
